Notice of Availability of Form 1095-B / 1095-C
Under the new federal law, the Paperwork Burden Reduction Act, Lincoln County Government is no longer required to automatically mail a copy of your annual Affordable Care Act (ACA) Form 1095-B (Health Coverage) or 1095-C (Employer-Provided Health Insurance Offer and Coverage) to all employees and covered individuals.
These forms provide information about the health coverage offered or provided to you and your family during the prior calendar year and are used for tax purposes.
You have the right to request a paper copy of your Form 1095-B or 1095-C at any time.
If you would like to receive a paper copy of your Form 1095-B or 1095-C, you may request one using any of the contact methods below:

Upon receiving your request, the form will be provided to you no later than 30 days after the date the request is received, or by January 31 of the following year, whichever is later.
Important Notes:
• This notice will remain posted on this website through October 15 of the current year for the prior calendar year's forms.
• Forms 1095-B and 1095-C are still being filed with the Internal Revenue Service (IRS) by [Company Name] as required by law.
• This change does not affect employees in states with their own individual health insurance mandates that require automatic paper distribution (e.g., California, Massachusetts, New Jersey, Rhode Island, Washington D.C.).
